History
Since 1995, Case Management of Michigan, Inc. has specialized in providing case management services for adults with severe and persistent mental illness or intellectual and developmental disabilities. We consistently coordinate care across multiple systems, including community and state hospitals, law enforcement, guardians, Community Mental Health (CMH) agencies, adult foster care (AFC) providers, and other treatment providers.
Our work grew out of the need to help individuals successfully transition from state institutions to less restrictive community placements.
We serve adults who often have a long history of psychiatric hospitalizations, multiple failed community placements, or have experienced treatment-resistant symptoms, including those with legal complications (e.g., Not Guilty by Reason of Insanity, Incompetent to Stand Trial).
We don’t believe people are difficult, but their circumstances can be. We work with individuals experiencing complex trauma, co-occurring medical conditions, and mental health symptoms that significantly impact their independence. Our work requires a coordinated team approach, much like managing complex medical diagnoses.
