Case Manager

Brief Job Description:

  • Provides targeted case management services to clients, assisting with tasks such as monitoring progress and supports, coordinating services and health care, making referrals to outside agencies for supported employment, therapy, etc.,
  • Identifies and provides emergency crisis services as necessary; makes immediate clinical assessments and responds according to accepted crisis intervention methods and techniques; coordinates other services as necessary.
  • Assess clients, evaluate the effectiveness of treatment plans and progress made by clients, create treatment plans and case reviews in collaboration with the client and treatment team.

Qualifications: LLBSW, LLMSW, LBSW, LMSW, or LLPC required for this position OR qualify as a QMHP (Qualified Mental Health Professiona) related human services degree (social work, community health, human services, psychology, criminal justice, etc.) nd QIDP (Qualified Intellectual Disability Professional)

Salary Range: 43,000 – 50,000
