Our comprehensive suite of services is designed to support individuals at every stage of their case management journey. From a thorough initial assessment to continuous care and professional support, we are dedicated to providing the highest level of services tailored to each individual’s unique needs.
Comprehensive Assessment
Our Clinical Director completes a comprehensive assessment prior to admission, reviewing historical documents, past services, current placement, strengths, needs, preferences, abilities, and barriers. We approach each case with curiosity and hope, looking for what has been missed or what is necessary for client success.
We work closely with referral sources and placements to ensure all necessary services can be provided.
The Clinical Director completes an Initial Service Plan, which the assigned case manager then reviews and expands upon with the client.
Assessment is an ongoing function of quality care. Our Case Managers are trained to continuously explore behaviors, situations, and new information to find creative solutions.
Intensive Ongoing Care
Our commitment is demonstrated through intensive case management contacts 2-4 times per month. These contacts support individuals and encourage positive problem-solving behaviors and adaptive functioning.
We work collaboratively with AFC homes, guardians, CMH agencies, and other treatment professionals to develop unique approaches for clients throughout the continuum of care.
Case Management of Michigan, Inc. also provides routine progress reports, 24-hour on-call services, coordinates psychiatric medication reviews, and provides behavioral assessments and plans.
Professional Staffing and AFC Homes
We understand the importance of having highly professional staff to meet the needs of our consumers and partners.
We provide linkage with other professional services as necessary.
Case Management of Michigan works with a variety of AFC Providers to offer environments tailored to each consumer’s needs.
