We have chosen to work with adults who are diagnosed with a mental illness. We have been asked to work with other populations in the past but have chosen to maintain our focus on adults to better serve their needs.
How do I get services for my loved one through Case Management of Michigan, Inc?
Case Management of Michigan, Inc. is a subcontracting service provider for many Community Mental Health agencies in the State of Michigan. Since we are a subcontracting agency, we cannot accept direct referrals, they must be initiated by the Access Center of the CMH Agency where your loved one is currently receiving services.
How often do you see your consumers?
At Case Management of Michigan, Inc. we believe that developing a strong relationship with our consumers is very important, consequently we strive to do visits with each person 2-4times per month. In some situations our contract may specify fewer visits.
Where do you provide services?
Our main office is located in Kalamazoo, Michigan. From this location we serve individuals placed in Allegan, Berrien, Cass, Calhoun, Kalamazoo, Kent, St. Joseph, and Van Buren Counties.
